Medication
Administration Guidelines |

The
The school staff is
prohibited from administering any medication to a pupil except as authorized by
the parent or guardian with:
1.
Written parental
consent for administering medication prescribed by the physician during school
hours.
2.
Parent or
guardian must obtain from the school nurse, a form for authorization to
administer prescribed medication. This
form must be completed by the family physician and returned to the school
nurse.
3.
The medication,
with the completed authorization form must be hand delivered by the parent to
the Health Room.
4.
The first day’s
dosage of any new medication must be given at home.
5.
Parent or
guardian must collect any unused medicine within one week after the expiration
of the physician’s order.
6.
Medication must
be in a labeled bottle from the pharmacist including the child’s name, name of
medication, dosage, and time to be administered.
Non-prescribed
or over the counter medicines may not be brought to school or administered by
the school staff, (Example: Tylenol, Advil) unless accompanied by physician’s
authorization.
No
medications will be given more than two times during the school day.
In
the event that your child should have a severe reaction to an insect sting,
epinephrine would be administered via injection by order of the school
physician.
