Required Documents Include:

  • Proof of residency
    •  A valid PA driver’s license, PA identification card or PA vehicle registration
    • Current lease or current rent payment receipts
    • Utility bill or utility turn on notice with current address
    • Pay stub with employer and employee's address
  • Birth Certificate
  • Immunizations
  • Student Residency Questionnaire/McKinney Vento Form
  • Home Language Survey
  • Residency Statement
  • Health History
  • Act 26 - Parent Registration Statement
  • Emergency Card
